Responsibilities

As our Homecare RN Clinical Manager, you will:

• Lead with Excellence:Oversee the homecare program in Union County, ensuring seamless coordination and delivery of patient care to uphold the highest standards of operational excellence.

• Drive Patient Care:Manage a caseload of homecare patients, offering direct care and support, and making a tangible difference in their lives.

• Expand Community Reach:Conduct outreach activities to improve community access to hospice care and enhance awareness of our services.

• Ensure Quality:Monitor and evaluate the quality and appropriateness of services, striving for exceptional patient satisfaction.

Qualifications

What We’re Looking For:

• Education:BSN Degree

• Qualifications:Registered Nurse (RN) with a valid license and relevant certifications.

• Experience:Proven experience in homecare or hospice care, with strong leadership and operational management skills.

• 2+ yearsof community health experience with at least 1+ year of prior experience with the agency

• Skills:Excellent clinical abilities, knowledge of homecare and hospice practices, and regulatory requirements. Ability to juggle multiple tasks while maintaining high care standards.

• Knowledgeof reimbursement and OASIS requirements.

• Attributes:Exceptional communication and interpersonal skills, with a focus on teamwork and patient-centered care.

Working Conditions/Physical Demand:Working indoors/outdoors with clinical equipment and use of phone and computer; client/customer contact
