RN Homecare Admission Nurse

Masonicare Home Health & Hospice - Norwich, CT

Day Shift / 32hrs/wk or 40hrs/wk

NEW PRODUCTIVITY LEVELS

The Homecare Admission Nurse ensures the quality and safe delivery of nursing and home health services. They coordinate the provision of home health care services that reflect the agency's philosophy and standards of home health and nursing care for assigned clients. The primary role is to admit patients to service, including medication reconciliation and completion of the OASIS data set. Typically, an Admission Nurse will handle 2-3 admissions per day and may perform revisits as needed to meet agency productivity goals.

Admission Nurse RN - ESSENTIAL RESPONSIBILITIES

  1. Carry out the plan of care for the client as developed by the nurse case manager, addressing all problems, identifying interdisciplinary care needs, and making recommendations for additional home health services.
  2. Assist the nurse case manager by coordinating the plan of care, maintaining continuity of care with other health professionals, communicating changes in patient status, and participating in client care conferences.
  3. Supervise patient care provided by Licensed Practical Nurses. Conduct supervisory visits and provide education and guidance to home health aides.
  4. Provide education, supervision, and counseling to clients and their families regarding home nursing procedures and care needs, utilizing agency educational materials as appropriate.
  5. Ensure timely and accurate documentation of patient and job information to facilitate the development of the OASIS data set, service orders, timesheets, and other required documents.
  6. Participate in the agency’s continuous quality improvement program.
  7. Communicate effectively and tactfully with clients, considering their age, cultural background, needs, abilities, and physical condition.
  8. Dependably report to work on time, use time off appropriately, and complete annual education and medical requirements.
  9. Perform other duties as required.

MINIMUM QUALIFICATIONS:

  1. RN Licensure
  2. Prior homecare experience, recent preferred
  3. OASIS certification is a plus
  4. Valid driver’s license and reliable transportation
