Responsibilities

Job Summary

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes as prescribed by the physician or authorized by the state/program, complying with laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and helps maintain continuity of care by exchanging information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, according to the care plan and in coordination with the care team from admit to discharge.
• Completes clinical nursing assessments as required by federal and state program requirements and payers.
• Ensures patient eligibility and medical necessity for services.
• Develops and revises individualized care plans in collaboration with community providers.
• Ensures care plan frequency and duration meet patient needs, revising as necessary with physician approval.

Education and Experience

Experience Desired

• Minimum of one year RN experience preferred.

License Requirements

• Current RN licensure in practice state.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
