Responsibilities

Job Summary

The Registered Nurse in Personal Care Services provides and directs nursing and personal care to patients in their homes as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals involved in the patient's care to ensure continuity, planning, and information exchange with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, following the care plan and coordinating with the care team from admission to discharge.
• Completes clinical nursing assessments per federal and state requirements and payer specifications.
• Ensures patient eligibility and medical necessity for services as defined by payer and agency policy.
• Develops and revises individualized care and service plans with community providers, in accordance with federal and state regulations.
• Ensures the care plan's frequency and duration meet patient needs and revises plans as necessary with physician approval.

Education and Experience

• Minimum of one year RN experience preferred.

License Requirements

• Current RN licensure in the practice state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
