Job summary

Avera Health’s Home Infusion Pharmacy in Sioux Falls is seeking an RN Case Manager to collaborate with the multidisciplinary care team to plan patient treatments and manage transitions of care. You will help optimize outcomes and coordinate resources across the continuum of care.

The role focuses on care coordination, patient education, and payer communications, with a regular day shift Mon-Fri, 8:00am-5:00pm. Join a compassionate team dedicated to Moving Health Forward.

Qualifications

• RN license in the state of practice is required.
• Ability to coordinate care across multidisciplinary teams.
• Strong communication with patients, families, and providers.

Responsibilities

• Liaise with hospitals and other health care services to provide treatment advice and coordinate payor communications.
• Identify community resources for non-covered benefits such as medications, transportation or equipment.
• Provide patient care and assist the provider as needed.
• Complete documentation, obtain insurance prior authorizations, and schedule procedures and treatments.
• Perform patient education and follow-up care with patients and families.
• Update policies and procedures relevant to the role.
• Attend discharge planning meetings to coordinate post-procedure needs.
