Job summary

Avera McKennan Hospital in Sioux Falls, SD is hiring a Transfer Center Case Manager RN to review medical necessity for transfers and coordinate patient movement within the hospital system.

You will work closely with physicians, case managers, and payors, using the EMR and patient-flow tools to ensure patients are placed in the right bed at the right time and that discharge plans progress smoothly.

Qualifications

• Licensed RN with active state license at hire.
• Ability to communicate effectively with patients, families, and staff.
• Experience coordinating care and coordinating transfers.

Responsibilities

• Review medical necessity for transfers and plan care transitions.
• Liaise between sending hospitals, case managers, and Avera Transfer Center staff.
• Utilize EMR and patient flow tools to place patients in appropriate beds and levels of care.
• Coordinate discharges and post-acute planning with physicians and payors.
• Support Continuity of care across the hospital and within Avera system.
