Job summary

Elara Caring is seeking a Registered Nurse Home Health to deliver high-quality in-home care across Chicago and western suburbs. You will assess patients, develop care plans, educate families, and coordinate with the care team while navigating HIPAA and safety standards.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate/Diploma/Associates/Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Must have valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy per HIPAA.
• Provide nursing care according to plan of care and physician orders.
• Monitor patient conditions and report changes to care team.
• Educate patients and families on care and safety.
• Assist in maintaining safe, quality, continuous patient care.
