McLeod Health seeks an experienced RN Clinical Manager for Home Health in Florence County. This on-site leadership role oversees daily operations and coordinates the interdisciplinary team to deliver high-quality home health services in the local area.

The position is full-time with an 80-hour biweekly schedule. You will supervise staffing, drive performance improvement, and ensure compliant, patient-centered plans of care in partnership with medical staff.

Qualifications

• 2+ years of experience in home health or hospice clinical operations.
• Licensed Registered Nurse with BLS certification.
• Must meet driving eligibility and own a valid SC driver's license.

Responsibilities

• Oversees daily operations and coordinates the interdisciplinary team to deliver high-quality home health.
• Organizes and facilitates interdisciplinary team care conferences for optimal care plans.
• Assumes 24-hour accountability for resources and patient care quality.
• Coordinates with Medical Staff to ensure plans of care are properly documented.
• Leads hiring, staffing, and performance management using KPIs and staffing metrics.
