Job summary

AMEDISYS HOME HEALTH in Florence, SC is seeking a full-time RN Case Manager for day shifts. You will perform patient assessments, develop care plans with the multidisciplinary team, and coordinate referrals to meet patient needs.

The role includes supervising LPNs and HHAs and timely documentation. Qualifications include 1+ year RN experience, active RN license for the state, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• 1+ year of clinical experience as an RN.
• Current RN license in the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.
• If <6 months RN experience, RN Intern program required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehab measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services.
• Participates in clinical development and continuing education.
