Job summary

The Home Health of Western Colorado is seeking a Registered Nurse (RN) to provide case management, coordination and supervision for in-home care clients in Montrose, CO. The role emphasizes nursing theory, patient assessments and adherence to the Nurse Practice Act to ensure high-quality home care services.

Two years of nursing experience (long-term care dementia care or acute care) are preferred, with an RN license in Colorado and CPR certification required.

Qualifications

• Graduate of an accredited school of professional nursing program; Baccalaureate degree preferred.
• Current license as a RN in the state of Colorado.
• Minimum of two years nursing experience, with a combination of long-term care dementia care or acute care. Home care experience preferred.
• CPR certification.

Responsibilities

• Reviews client referral information and responds to requests and inquiries as appropriate.
• Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
• Directs, plans and initiates appropriate action independently and responsively in-home care situations.
• Performs job duties in accordance with agency policies, procedures, and professional and community standards.
• Performs other job duties as assigned by Director of Clinical Services.
