Volunteers of America National Services (VOANS) is seeking a Registered Nurse in Montrose, CO to join Home Health of Western Colorado. The role involves case management, coordination, and in-home nursing care to clients in a compassionate, mission-driven environment.

The RN will hold Colorado licensure, have at least 2 years of nursing experience (preferably with home health, dementia care or acute care), and CPR certification.

• Current license as a RN in the state of Colorado.
• Minimum of two years of nursing experience, with a combination of long-term care dementia care or acute care.

Responsabilidades

• Provides case management, coordination, supervision and implementation of professional and supportive services to clients.
• Performs client assessments based on agency standards of practice to ensure effective home care services.
• Directs, plans and initiates appropriate action independently in in-home care situations.
• Performs duties in accordance with agency policies and professional standards.
• Performs other duties as assigned by Director of Clinical Services.

Pay Range: $39 - $46.50 per hour based on previous work experience.

Schedule: Fulltime Monday - Friday, 8-hour shifts, occasional weekends

What You Bring (Requirements)

• Graduate of an accredited school of professional nursing program; Baccalaureate degree preferred.
• Current license as a RN in the state(s) of Colorado.
• Minimum of two (2) years of nursing experience, with a combination of long-term care dementia care or acute care. Home care experience preferred.
• CPR certified.

What You’ll Be Responsible For

The Registered Nurse (RN) provides case management, coordination, supervision and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act.

• Reviews client referral information and responds to requests and inquiries as appropriate.
• Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
• Directs, plans and initiates appropriate action independently and responsively in-home care situations.
• Performs job duties in accordance with agency policies, procedures, and professional and community standards.
• Performs other job duties as assigned by Director of Clinical Services.

The Home Health of Western Colorado is a Medicare/Medicaid-certified home health care agency in Montrose, CO which include experienced team of nurses, rehab therapists and home health aides dedicated to providing individualized health care needs with compassion and empathy. We are trained in caring for older adults, providing in-home health care services in the Delta, Montrose and Ouray counties and Montrose, CO.
