Job summary

Elara Caring is seeking a Registered Nurse Home Health in Oklahoma to deliver compassionate, in-home care across patient families. You will assess needs, follow care plans, educate families, and coordinate with the care team.

Requirements include an active RN license, nursing degree, and 1+ year of clinical experience. Home Health experience is preferred. Must be able to lift up to 50 pounds and have reliable transportation to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, AD, or BSN).
• 1+ year nursing experience in a clinical setting.
• Home Health experience preferred but not required.

Responsibilities

• Assess patients to determine care needs and plan.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy per HIPAA.
• Provide nursing care per plan and orders.
• Monitor conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Take action when patient conditions change.
• Support high-quality, safe patient care.
