Job summary

Sentara Home Care in Chesapeake, VA is seeking a qualified RN to join our home health and hospice team. The Direct Care Registered Nurse will develop, implement, and evaluate plans of care for diverse patient populations while coordinating with a multidisciplinary team.

This full-time, day-shift role requires an active VA RN license, BLS within 90 days, and an Associate's Degree in Nursing, with at least one year of nursing experience; home health/hospice experience is preferred.

Qualifications

• Active RN license in Virginia.
• BLS certification within 90 days of hire.
• Associate's Degree in Nursing required.

Responsibilities
