The position requires an RN license in Virginia, BLS within 90 days, and an Associate's Degree. Prior home care/hospice experience is preferred, with a stable daytime schedule in a full-time capacity.

Qualifications

• Associate degree in nursing is required.
• Active RN license in Virginia.
• BLS certification required within 90 days of hire.
• 1 year of nursing experience preferred; Home Care/Hospice experience preferred.

Responsibilities

• Develops, implements, and evaluates plans of care for specified patient populations.
• Manages a patient caseload through coordination with a multidisciplinary team to provide integrated care.
