Job summary

Heartland Home Health seeks an experienced Home Health RN Executive Director to lead a branch with a focus on quality care, compliance, and team development. You will oversee admissions, clinical operations, and performance management to ensure excellent patient outcomes.

The role requires an active RN license, 2+ years in healthcare leadership, and strong Medicare knowledge. You will partner with physicians and community partners to strengthen referrals and drive operational excellence.

Qualifications

• RN license in the state of employment.
• Associate degree in nursing or related field.
• 2+ years in home health, hospice, or healthcare leadership.
• Strong understanding of Medicare Conditions of Participation and home health regulations.
• Demonstrated ability to lead teams, manage clinical/administrative operations, ensure compliance.
• Excellent interpersonal, communication, and leadership skills.
• Proficient with EMR systems and scheduling software.
• Valid driver’s license, reliable transportation, and CPR certification.

Responsibilities

• Direct day-to-day operations of the home health branch, ensuring compliance with CMS, Medicare, Joint Commission, etc.
• Oversee patient admissions and discharge planning for a seamless patient experience.
• Provide clinical oversight to interdisciplinary home health staff (RN Case Managers, LPNs, therapists, aides, admin).
• Interview, hire, train, supervise, and evaluate staff.
• Promote best practices in skilled nursing and chronic disease management.
• Monitor operational efficiency, service delivery, and patient satisfaction.
• Lead performance management: evaluations, scheduling, workload distribution.
• Ensure survey readiness and participate in QAPI initiatives.
• Manage branch budgeting, payroll, supply costs, and referral development.
• Build relationships with physicians, discharge planners, and community referrals.
