The Clinical Team Manager is an RN position responsible for management and supervision across clinical disciplines including skilled nursing, home health aide, physical therapy, occupational therapy, speech therapy, and medical social services. Key duties include coordinating patient care activities, ensuring service quality, staff scheduling, monitoring performance, staff orientation, and participating in committees to improve operations. The role involves verifying physician orders, managing communication with referral sources, on-call duties, and working under supervision of the Clinical Team Manager Lead or Director of Nursing.

Position Highlights & Primary Responsibilities:

  1. Manage recruitment, hiring, and development of clinical and non-clinical staff to ensure quality care 24/7.
  2. Participate in orientation and training of staff.
  3. Ensure supervisory requirements are met timely.
  4. Lead effectively, solve problems efficiently, and analyze facts accurately.
  5. Ensure clinical assessments, physician communication, service planning, and discharge planning are performed proficiently.
  6. Assign staff, monitor schedules, match qualifications to patient needs, and manage quality through care appraisals and supervision.
  7. Maintain documentation to meet legal, regulatory, and reimbursement requirements.
  8. Develop knowledge of services and resources, directing staff to meet patient needs.
  9. Ensure care plans are executed as written and reassessed appropriately.
  10. Conduct staff evaluations and ongoing skill assessments.
  11. Supervise clinical processes, ensure proper documentation, and support reimbursement efforts.
  12. Participate in interdisciplinary care conferences, monitor documentation accuracy, and investigate complaints.
  13. Perform home visits as needed, recognize emergencies, and participate in accreditation and quality improvement initiatives.
  14. Assist in developing performance improvement reports and serve as a resource for clinical staff.
  15. Ensure compliance with documentation standards, third-party requirements, and safety protocols.
  16. Promote agency philosophy, maintain confidentiality, and communicate positively with patients and staff.
  17. Meet productivity standards, follow the Patient's Bill of Rights, and adhere to infection control policies.
  18. Demonstrate flexibility, teamwork, and a commitment to professional development.

Required Qualifications:

  • Graduate of an accredited nursing school
  • Current RN licensure in Texas
  • Certification in Basic Cardiac Life Support (BLS) Course C
  • 3+ years RN experience in acute or medical/surgical settings
  • Knowledge of regulations, Medicare/Medicaid, and company policies
  • Familiarity with medical terminology and equipment
  • Proven ability to practice independently and possess good organizational and communication skills
  • Reliable transportation and a valid driver’s license
  • Ability to travel and meet health screening requirements

Preferred Qualifications:

  • Experience in community or home health settings
  • Supervisory experience
  • Proficiency in office procedures and computer skills
