Qualifications

• Current, unrestricted Connecticut RN license.
• Associate degree in a nursing-related field.
• At least two years of RN experience in home health or hospice.
• One year of supervisory experience preferred.

Responsabilités

• Lead and support your clinical team, coaching direct care staff.
• Oversee staff assignments, referrals, care coordination, and timely starts of care.
• Review OASIS documentation and Plans of Care for accuracy and appropriateness.
• Coordinate services with patients, families, physicians, providers, and facilities.
• Support branch success by partnering with leadership and providing field care as needed.
