Job summary

Patriot At Home in Fairlawn, OH is seeking a full-time Registered Nurse for home health care to coordinate patient care across the community. You will evaluate nursing service effectiveness, perform admissions, transfers, recertifications, and discharges for home care patients, and provide care in patients' homes while adhering to Medicare and Medicaid guidelines.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• One year or more of community/home health or hospital experience preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check.
• Current CPR (BLS) certification.

Responsibilities

• Coordinate total patient care.
• Evaluate nursing service effectiveness for patient and family.
• Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient.
• All other duties as assigned.
