Job summary

Phoenix Home Care in Missouri is seeking compassionate RNs for home health. You will manage your own caseload with supervision of visits, ensuring high-quality patient care in diverse weekly visits and starts of care.

Responsibilities include patient education on CHF, COPD, Diabetes, wound care, and medication management, plus collaboration with physicians to create diagnosis-driven plans. A supportive therapy team is available to aid in patient recovery.

Qualifications

• Active Missouri or Compact RN license is required or pending license transfer.
• Home health experience strongly preferred.
• Compassionate communicator with patients, families, and physicians.
• Strong clinical skills in physical assessment and diagnostic interpretation.

Responsibilities

• Manage your own caseload including supervision of visits — you run your schedule.
• Varied weekly visits including routine care, evaluations, and starts of care.
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care.
• Work alongside a therapy team including PT, OT, and Speech.
