Job summary

Phoenix Home Care and Hospice is seeking a Home Health Case Manager to coordinate care directly in patients' homes across Lake Ozark, MO and surrounding areas. You will supervise LPN visits, manage a varied caseload, and educate patients on conditions like CHF, COPD, diabetes, wound care, and medication management.

Collaborate with physicians and a strong therapy team (PT/OT/Speech) to develop diagnosis-driven plans of care.

Qualifications

• Missouri or Compact RN license required.
• Driver's license required and vehicle with current auto insurance.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Varied weekly caseload includes routine visits, evaluations, and starts of care.
• Educate patients on CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Communicate with physicians to obtain orders and write diagnosis driven plans of care.
• Work alongside strong therapy team including PT/OT/Speech.
