Job summary

Freedom at Home is seeking compassionate Registered Nurses to join our home health team in a dynamic and supportive environment. We offer Full-Time, Part-Time, and PRN roles with excellent pay, paid mileage, and flexible scheduling to help clinicians balance patient care with personal life.

As an RN with Freedom at Home, you will perform skilled visits, develop and update Care Plans, complete OASIS and documentation on time, educate patients and families, and collaborate with physicians and the

Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current, unrestricted Ohio Registered Nurse license.
• Current CPR certification.
• Minimum of one year of nursing experience required.
• Home Health experience is preferred but not required.
• OASIS experience is a plus.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
• Develop, implement, and update individualized Plans of Care.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Supervise Home Health Aides in accordance with state and federal regulations.
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
