Job summary

LCMC Health is seeking an RN Home Health Care Manager in New Orleans to lead a compassionate team delivering high-quality home health care. The role focuses on clinical and operational leadership, ensuring safe, patient-centered services in patients’ homes, and coordinating with physicians and case managers for seamless transitions of care.

The ideal candidate will have a Louisiana RN license, BLS certification, and at least five years of nursing leadership experience within home health

Qualifications

• Louisiana RN license required.
• AHA BLS certification required.
• BSN preferred.
• Minimum five years of professional nursing experience.
• Health Home Care, Home Health, Case Management or Nursing Leadership experience preferred.
• Strong knowledge of Medicare regulations, OASIS documentation, COP and home health standards.
• Demonstrated leadership, communication, problem-solving, and team-building skills.
• Ability to lead quality improvement initiatives while balancing patient care with operations.

Responsibilities

• Lead the daily clinical and operational activities of the Health Home Care department.
• Supervise a multidisciplinary team of nurses, therapists, and other staff providing care in patients' homes.
• Ensure delivery of safe, evidence-based, patient-centered home health services.
• Coordinate and participate in ongoing OASIS training for staff.
• Manage staffing, scheduling, hiring, onboarding, performance evaluations, coaching, and development.
• Maintain accountability for agency policies, nursing standards, Medicare COP, state regulations, and accreditation requirements.
• Partner with physicians, case managers, referral sources, and healthcare leaders to coordinate transitions of care.
• Monitor quality metrics, patient outcomes, and satisfaction while leading QA and performance improvement initiatives.
• Promote patient safety by maintaining a culture of accountability and timely completion of safety reports.
• Participate in budget planning while managing department productivity and cost containment.
• Implement evidence-based practices to improve clinical outcomes and operational efficiency.
• Develop, review, and update departmental policies and procedures to ensure regulatory compliance.
• Foster a collaborative, positive work environment supporting engagement and growth.
• Maintain relationships with organizational leaders and participate on interdisciplinary committees.

Your Everyday

The Must-Haves

• Current Louisiana Registered Nurse (RN) license.
• Current American Heart Association BLS Certification.
• Bachelor's degree in Nursing (BSN) preferred.
• Minimum of five (5) years of professional nursing experience.
• Previous Health Home Care, Home Health, Case Management, Clinical Supervisor, or Nursing Leadership experience preferred.
• Strong knowledge of Medicare regulations, OASIS documentation, Conditions of Participation, and home health compliance standards.
• Demonstrated leadership, communication, problem-solving, and team-building skills.
• Ability to lead quality improvement initiatives while balancing patient care excellence with operational and financial performance.
