Pay range for this position is $40.94 - $59.92/hour. Actual pay is based on years of licensure. This is an hourly position.
Eligible for a $20,000 Sign-on Bonus with a two-year work commitment to OSF Home Care - external candidates
$10,000 Sign-on Bonus for New Grad RNs with a two-year work commitment to OSF Home Care – internal and external candidates
OSF will invest in you from day 1! Candidates with 5+ years of direct experience will receive a bonus of 24 hours of vacation/holiday time frontloaded on day 1. This is in addition to our generous paid time off plans!
*Weekend differential of $3.25/hr.*
POSITION SUMMARY: The RN Home Health collaborates with the RN Case Manager to develop and implement the home health plan of care. The RN communicates with the patient’s provider and other care team members such as nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment, and home infusion services to facilitate and expedite the best possible patient outcomes. The RN Home Health is responsible for a scope of care that encompasses all ages from infancy through geriatrics and demonstrates the knowledge and skills necessary to provide age-appropriate patient care. The RN provides evidence-based, patient-centered care at the patient’s residence, utilizing critical thinking and decision-making skills. The RN has a thorough understanding of OASIS documentation and plan of care development, impacting 5 Star Agency Performance and Value-Based Purchasing performance.
REQUIRED QUALIFICATIONS:
Education: Completion of a nursing program
Licensure/Certification: IL or MI RN Licensure depending on work location
Current American Heart Association Healthcare Provider BLS certification is required prior to start date.
Other Skills/Knowledge: Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft Office software.
Strong analytical and problem-solving skills, with attention to detail.
Provides own transportation and auto insurance, and abides by all laws, rules, and recommendations for safe driving while on duty.
Maintains a valid driver’s license.
PREFERRED QUALIFICATIONS:
Education: BSN
Experience: Experience as an RN in a healthcare environment; Home Health OASIS experience preferred.
Licensure/Certification: OASIS Certification
Other Skills/Knowledge: Basic proficiency with Electronic Medical Records and Microsoft applications
