Job summary

Freedom at Home is seeking a compassionate and dedicated Registered Nurse (RN) to join our growing team in Muskegon, Michigan. This role focuses on delivering high-quality home health care, performing skilled nursing visits, and coordinating with physicians and the interdisciplinary team.

You will educate patients and caregivers, develop plans of care, and ensure accurate OASIS documentation while supporting a positive, clinician-focused culture.

Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current, unrestricted Michigan Registered Nurse license.
• Current CPR certification.
• Minimum of one year of nursing experience.
• Home Health experience is preferred but not required.
• OASIS experience is a plus.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
• Develop, implement, and update individualized Plans of Care.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Supervise Home Health Aides in accordance with state and federal regulations.
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
