Job summary

CommonSpirit Health at Home is seeking an experienced Home Health Registered Nurse to deliver high-quality care in the patient’s home environment in Fort Wayne, Indiana. This role emphasizes personalized nursing care, with a focus on maintaining patient independence and wellbeing.

You will develop and adjust comprehensive care plans, collaborate with a multidisciplinary team, and educate patients and families. Current RN licensure in Indiana, CPR/BLS, and a valid driver's license are required.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

• Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
