Description:This is an office-based position as Patient Care Manager in Home Health.The role involves overseeing the dashboard, reviewing orders, and maintaining contact with physicians and clinicians.The candidate will manage new admissions and may assist with scheduling.Occasional field visits outside the office may be required.Patient population includes adults to geriatrics.The ideal candidate should have experience in administration of blood products, admission assessments, and care management of IVs and central lines.Skills in clinical assessment documentation, OASIS, infection control precautions, and wound care are also essential.Leadership and managerial experience are preferred.The candidate must be fully compliant to start on time.Call requirements may include possibly one per week as needed.The candidate may be required to work major holidays as per the assignment calendar.If the candidate does not have experience with HCHB, they will need to complete an online course prior to starting.
