Job summary

Erickson Senior Living's Maris Grove in Glen Mills, PA is hiring Registered Nurses for Home Care. The RN provides nursing care per policy, develops and supervises an individualized service plan, observes residents, and coordinates with physicians and families.

You will supervise Home Support team, deliver care per the plan, participate in care conferences, and train aides. Requirements: at least 6 months RN experience; senior care experience preferred; basic computer skills and EMR experience

Qualifications

• Must have a minimum of 6 months’ RN experience.
• Experience in caring for seniors and/or cognitive impairment preferred.
• Basic computer skills including MS Office, internet and web applications.
• Experience with an electronic medical record is preferred.

Responsibilities

• Evaluate residents’ needs and develop an individualized service plan with the team.
• Communicate changes in residents’ condition.
• Ensure the resident’s plan is followed by the Home Support team.
• Deliver nursing care per the plan and nursing standards.
• Participate in care conferences and resident meetings.
• Provide orientation and training for other agency nurses and aides.

Location: Maris Grove by Erickson Senior Living

Join our team as Registered Nurses for Home Care. The RN in Home Care provides nursing care according to established policies and procedures for a home care agency. The RN develops and supervises the implementation of a resident’s individualized service plan, observes the resident’s condition, and communicates to the Physician and family/responsible party as needed. The RN oversees the resident’s care delivered by the Home Support team of LPN’s, Aides, and Home Care workers and companions. Compensation:$up to $48.00 per hour, based on experience.

How you will make an impact

• Partnering and collaborating in the initial evaluation to assess the residents’ needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitioner
• Communicating any changes in the resident’s condition
• Ensuring that the resident’s individualized plan is implemented and followed by the Home Support agency team of providers
• Delivering nursing care as outlined in the plan of care and according to acceptable nursing standards
• Participating in care conferences and other resident–related meetings
• Participating in the orientation and training of other agency nurses as well as training any Aide staff development activities

What you will need

• Must have a minimum of 6 months’ experience as an RN
• Experience in caring for seniors and/or senior with cognitive impairment preferred
• Basic computer skills required including experience with Microsoft Office, internet and web applications
• Experience with an electronic medical record is preferred
