Job summary

Maris Grove by Erickson Senior Living is seeking an Registered Nurse for Home Care. The RN provides nursing care per policy in a home setting, develops resident service plans, observes conditions, and coordinates with physicians and families. Compensation: up to $48.00 per hour, based on experience.

Qualifications

• Minimum 6 months’ RN experience.
• Experience caring for seniors and/or cognitive impairment preferred.
• Basic computer skills including MS Office, internet and web applications.
• EMR experience preferred.

Responsibilities

• Assess residents and develop/implement individualized service plans.
• Communicate changes in residents’ condition to physicians and families.
• Ensure plan is followed by Home Support team and providers.
• Deliver nursing care per plan and nursing standards.
• Attend care conferences and resident-related meetings.
• Participate in orientation and training of agency nurses and aides.

Location: Maris Grove by Erickson Senior Living

Join our team as Registered Nurses for Home Care. The RN in Home Care provides nursing care according to established policies and procedures for a home care agency. The RN develops and supervises the implementation of a resident’s individualized service plan, observes the resident’s condition, and communicates to the Physician and family/responsible party as needed. The RN oversees the resident’s care delivered by the Home Support team of LPN’s, Aides, and Home Care workers and companions. Compensation:$up to $48.00 per hour, based on experience.

How you will make an impact

• Partnering and collaborating in the initial evaluation to assess the residents’ needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitioner
• Communicating any changes in the resident’s condition
• Ensuring that the resident’s individualized plan is implemented and followed by the Home Support agency team of providers
• Delivering nursing care as outlined in the plan of care and according to acceptable nursing standards
• Participating in care conferences and other resident–related meetings
• Participating in the orientation and training of other agency nurses as well as training any Aide staff development activities

What you will need

• Must have a minimum of 6 months’ experience as an RN
• Experience in caring for seniors and/or senior with cognitive impairment preferred
• Basic computer skills required including experience with Microsoft Office, internet and web applications
• Experience with an electronic medical record is preferred
