What it's like to be a FFS Home Care RNYou will play a pivotal role in delivering and coordinating comprehensive multidisciplinary home health services to patients within the community. Collaborating closely with physicians and team members, you will implement tailored care plans while ensuring services are cost-efficient and aligned with insurance requirements. Your responsibilities will encompass assessing patient needs, completing initial evaluations, and delivering direct care in patients' homes or other facilities. You will accurately complete OASIS assessments, manage diagnosis coding, and engage in weekly PPS meetings to report on service adherence. Your expertise will guide you in educating patients and their families on disease management and self-care strategies, ensuring they receive optimal support throughout their care journey.
Requirements for this RN-Home Care Nurse-FFS jobYou should possess a minimum of one year of nursing experience from an accredited institution, with a preference for candidates with a background in long-term care or Certified Home Health Agency (CHHA) settings. While experienced nurses are valued, we welcome new graduates, providing the necessary training to ensure success. Strong critical thinking and problem-solving skills are essential, as you will navigate various home environments that may be unsafe or unsanitary.
Reliable transportation is crucial, along with the ability to travel efficiently within Brooklyn, Queens, and Manhattan based on case assignments. The role demands physical stamina, including the ability to stand, walk, bend, and lift up to 50 pounds unassisted or 325 pounds with assistive devices. Proficiency in relevant software tools for documentation and communication is also necessary to effectively coordinate patient care and ensure accurate record-keeping.
