Job summary

Mercy is seeking an experienced Registered Nurse to manage home health patients in Oklahoma City, aligning with our standards of community health nursing. You will deliver high-quality patient care, coordinate plans of care, and collaborate with a multidisciplinary team to ensure optimal outcomes.

The role emphasizes case management, documentation accuracy, and adherence to agency philosophy. BSN preferred; two years of Medical/Surgical experience is required.

Qualifications

• Must be a graduate of an accredited registered nursing program.
• Current RN licensure in Oklahoma.
• Two years of Medical/Surgical nursing experience.
• Preferred: BSN.
• Home health experience preferred.

Responsibilities

• Provide case management and home nursing services to patients per agency standards.
• Coordinate care, document care plans, and collaborate with clinical team.
