Job summary

Old Colony Elder Services (OCES) seeks a dedicated Registered Nurse for our Home Care Program to assess and authorize services, promoting coordination and high quality, cost-effective care for older adults and individuals with disabilities.

You will work with a multidisciplinary team to determine eligibility, establish care plans, and ensure services meet funding source regulations. Strong communication and organizational skills are essential for success in this role.

Qualifications

• Excellent verbal and written communication skills.
• Excellent interpersonal and conflict resolution skills.
• Excellent organization skills and attention to detail.
• Strong analytical and problem-solving skills.
• Proficient with Microsoft Office Suite or related software.
• Extensive knowledge of developing assessment care plans.
• Two years’ clinical/community-based experience.

Responsibilities

• Determine consumers’ eligibility for programs and services according to procedures and guidelines.
• Establish, review and revise care plans in accordance with funding source regulations and OCES policies.
• Collaborate with care management team to determine service options and authorize packages.
• Maintain records and prepare reports as required.
• Provide consultation to providers and community agencies; participate in trainings.
