Job summary

NeighborHealth’s PACE Winthrop location is seeking a Registered Nurse to provide direct patient care, education, and coordination within a community health setting. This role emphasizes patient history assessment, test administration, and collaboration with providers to implement care plans.

The ideal candidate holds MA RN licensure, a BSN preferred, and BCLS certification with 1 year of relevant nursing experience in community health.

Qualifications

• Bachelor of Science in Nursing preferred.
• Completion of Accredited Nursing Program.
• Licensure in Massachusetts as an RN.
• BCLS certification per department policy.
• 1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.

Responsibilities

• Assess a patient through obtaining patient history and performing appropriate tests and observations.
• Communicate with the patient’s provider and educate the patient on test results, new care plans and/or other relevant health care information including the use of newly prescribed medications and/or changes in the current medication regime.
• Demonstrates sound clinical judgment/decision making when triaging sick patients, both on the phone and in the waiting room.
• Safely and effectively administers medications, tests and treatments as ordered.
• Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.
