Job Title: Registered Nurse

Department: Home Care

Location: ElderOne- 800 Emerson St, Rochester, NY 14613

Hours Per Week: 40 Hours

Schedule: Monday-Friday 8am-4:30pm- 5-7 weekends/yr.

RESPONSIBILITIES

• Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care.
• Communication. Counsel and guide individuals and families towards self‑help in recognition and solution of physical, emotional and environmental problems.
• Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff.

REQUIRED QUALIFICATIONS

• Nursing Diploma required, ASN preferred.
• License and current registration to practice as Registered Professional Nurse in New York State.
• Current NYS Driver’s license.

PREFERRED QUALIFICATIONS

• Bachelors degree of Science in Nursing preferred.
• 1 year of experience working with a frail or elderly population preferred.
• 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.
• Availability of Motor Vehicle.

PHYSICAL REQUIREMENTS

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE

$75,733.00 - $101,014.00
