Job summary

Sarah Bush Lincoln is seeking a Registered Nurse for Home Care to provide patient-focused nursing in home settings, following physician orders and state licensure guidelines. You will work under RN supervision to implement care plans, assess needs, and coordinate services for safe, effective patient outcomes.

You will document care activities, collaborate with families, and arrange supplies while ensuring compliance with regulations and agency policies.

Qualifications

• High School diploma required; RN licensure Illinois.
• BLS/CPR certification within 90 days of hire.
• Valid driver's license required for in-home visits.

Responsibilities

• Assist with plan of care implementation under RN supervision.
• Evaluate patient needs and refer to RN Case Manager as needed.
• Order supplies specific to patient care and coordinate home visits.
• Document in patient records reflecting quality home health delivery.
• Coordinate care with patient and family for planning and discharge.
• Provide equipment and supply information to patient and family.
• Work independently in patient homes per Nurse Practice Act and physician orders.

RN (Home Care) Job Description

Registered Nurse is responsible for providing nursing care in accordance with state licensure status and in compliance with policies and procedures, applicable health care standards and governmental laws and regulations.

Responsibilities

• Assists with implementation of the plan of care under the direction and supervision of the registered nurse.
• Evaluates patient/client needs and makes appropriate referrals to the RN Case Manager for other services.
• Orders all supplies patient specific to meet patient needs.
• Other duties as assigned to assist with overall operations of the agency.
• Participates in-home health care case conferences, staff meetings and in-services as required.
• Prepares documentation in patient/client clinical record to reflect delivery of quality and safe home health care. Documentation is submitted timely and according to policies/procedures.
• Provides for patient/client and family input into care planning and discharge planning. Documents collaboration with care plan and discharge planning.
• Provides information regarding equipment and supply needs to patient/client/family and makes appropriate referral.
• Works in the patient home, independently within the standards of the Nurse Practice Act and following physician orders, to coordinate the patients’ care in a safe manner.

Compensation

Estimated Compensation Range $39.12 - $60.64. Pay based on experience.
