Responsibilities

• Develops and implements individual plans of care in collaboration with other members of the healthcare team in accordance with the established guidelines and standards of nursing care.
• Promotes patient and family centered care in a healing environment.
• Executes nursing interventions by collaborating with the interprofessional team incorporating appropriate standards of care and practice and patient outcomes.
• Assesses and systemically collects data related to patient health status and works with the care team to develop the plan of care.
• Evaluates patient's response to plan of care and collaborates with the interprofessional team to adjust as needed.
• Assist with coordination of care across the continuum (inpatient, outpatient, home, SNF, schools).
• Assist with management of patients with multiple chronic conditions, acute episodes, behavioral health challenges while promoting prevention and wellness.
• May float to other clinics as needed for operations.
• Performs other responsibilities as assigned.
