The role emphasizes patient education, care coordination, and collaboration with cardiology and electrophysiology teams.

Requirements include a BSN, 3+ years RN experience or equivalent, and strong communication and clinical judgment.

Qualifications

• Minimum of three years RN experience or equivalent with RN role required.
• BSN is required.
• Effective verbal and written communication skills required.
• Ability to prioritize and make independent clinical judgments.
• Experience with basic patient care equipment and policy manuals.
• Collaborative with an interdisciplinary team.
• A Nebraska RN license or compact state license is required.
• BLS certification from an AHA-endorsed program.

Responsibilities

• Coordinate a patient care delivery system to achieve high quality, cost-effective outcomes across care continuum.
• Provide targeted interventions to avoid hospitalization and ER visits.
• Assist complex patients in transition of care across settings.
• Position may be within a primary or specialty care area.
