Personal Touch Home Health Care Services Inc. is seeking a licensed RN to provide professional nursing care within an interdisciplinary team in a home health setting.

You will assess, plan, implement, and evaluate patient care using the nursing process. You will develop individualized care plans with patients and families, implement interventions, document progress, and educate patients tailored to their age-specific needs.

Qualifications

  • Active RN license in good standing.
  • BLS certification within 30 days of hire.
  • Bachelor’s degree in nursing; HS diploma with relevant experience accepted.
  • Knowledge of evidence-based nursing practices and patient safety.
  • Strong clinical judgment and ability to work collaboratively.
  • Proficiency with medical equipment and patient care procedures.
  • Excellent communication and patient education skills.

Responsibilities

  • Assess patient status through interviews, observations, and exams.
  • Develop individualized care plans collaboratively with patients, families, and healthcare team members.
  • Implement interventions to meet patient outcomes and document progress regularly.
  • Modify care plans based on ongoing evaluations and changes in patient conditions.
  • Complete accurate documentation, including charts and event reports.
  • Maintain professional accountability by seeking resources and pursuing self-development.
  • Operate and troubleshoot medical equipment, ensuring safety and proper use.
  • Perform bedside lab testing and quality control following medical and hospital protocols.
  • Collaborate with the team to organize patient care activities from admission to discharge.
  • Assess and tailor patient/family education to their age-specific needs.
  • Participate in data collection, analysis, and process improvements to enhance patient satisfaction and safety.

  • Strong clinical judgment, problem-solving skills, and ability to work collaboratively.
  • Technical proficiency with medical equipment and patient care procedures.
  • Effective communication and patient education skills.
