Piedmont Athens Regional Hospital in Georgia is seeking a Heart Failure Nurse to join the Heart Failure Integrated Practice Unit (IPU). The nurse delivers expert care using the nursing process to assess, plan, implement and evaluate patient-centered care across the continuum.

Qualifications

  • Graduate of a nursing program.
  • Current Georgia RN license or NLC/eNLC multistate license; BLS certification required.

Responsibilities

  • Provide expert nursing care using the nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum.
  • Coordinate patient care activities within the Heart Failure IPU.
  • Collaborate with the interdisciplinary team to ensure safe, effective, and high-quality care.
  • Support education, transition planning, and long-term disease management for heart failure patients.
  • Contribute to reducing readmissions and improving symptom control and quality of life.

Overview

The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. In 2020, our program distinguished itself as thethird in the nationto achieveHeart Failure Certification through DNV, reflecting commitment to evidence-based standards and a culture of continuous improvement. In 2022, we furtherdemonstratedour commitment to exceptional outcomes by achievingGold Plus Status from the American Heart Association, a designation we continue tomaintainthrough sustained performance in quality metrics and guideline-directed care.

Our multidisciplinary team plays a critical role in advancing patient outcomes through comprehensive education, individualized transition planning, and long-term disease management. These efforts directlyimpactreduced readmissions, improved symptom control, and enhanced quality of life for patients living with heart failure. The heart failure nurse is central to this work, providing expert nursing care byutilizingthe nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum.

Practicing within the framework of organizational policies and procedures, the heart failure nursedemonstratesprofessional accountability, clinical leadership, and a commitment to ongoing growth. This roleis responsible forupholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinicalexpertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery ofexceptional outcomes for our patients.

Minimum Education Required

Graduate of a nursing program

Minimum Experience Required

New Graduates of a nursing program eligible

Minimum Licensure/Certification Required By Law

Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License. BLS certification required.

Additional Qualifications

Advanced certification in field of specialty, if applicable. Bachelor\'s degree preferred. Nursing Experience in Hospital Setting Preferred.
