Job summary

Siouxland Community Health Center is hiring an RN Health Coach to support chronic disease management and patient education. The role requires active RN licensure, BLS, and case management experience, with the potential for Certified Diabetes Educator credentials within 6 months.

The successful candidate will educate patients, set measurable self-management goals, coordinate with insurance for authorizations, and collaborate with care teams in a clinic setting.

Qualifications

• RN license is required; BSN preferred.
• Current BLS and Mandatory Reporter certifications.
• Clinical office and case management experience preferred.
• Certified Diabetes Educator certification or eligibility within 6 months.

Responsibilities

• Educate patients on chronic disease management and prevention.
• Assist patients with identifying and setting proper self-management goals including coordination to determine an appropriate goal achievement.
• Contact insurance companies for preauthorization/precertification approvals for doctor visits, procedures, diagnostic tests, etc., and document all preauthorization activities in the EMR.
• Instruct diabetic patients on proper use of diabetic meters, insulin instructions, and downloading meter results.
• Promote public awareness and education in preventive health habits of SCHC patients via ensuring proper screenings are completed.
• Collaborate with provider teams regularly, actively participating in daily huddles.

RN Health Coach

1 - Full-time opening

schedule to include 4-day work week : ( 3 ) 8hr shifts & (1) 12hr shift)

Successful candidate must have solid experience & advanced knowledge of:

• Minimum of RN license, BSN preferred.
• Current BLS and Mandatory Reporter certifications.
• Clinical office and case management experience preferred.
• Certified Diabetes Educator certification or ability to obtain within 6 months of employment.
• Experienced RN with a background in case management who is self motivated and capable of working with others to improve patient care.

Successful candidate must be able to perform primary functions of position:

• Educate patients on chronic disease management and prevention
• Assist patients with identifying and setting proper self-management goals including coordination to determine an appropriate goal achievement
• Contact insurance companies for preauthorization/precertification approvals for doctor's visits, procedures, diagnostic test, etc. Including maintaining detailed records in the EMR of all preauthorization activities.
• Instruct diabetic patients on proper use of diabetic meter, insulin instructions, and knowledge of downloading meter results
• Promote public awareness and education in the preventative health habits of SCHC patients via proactively assuring proper screenings are completed
• Collaborate with provider teams on a regular basis, ensuring to be an active part in daily huddles
• These functions are not all inclusive
