Job summary

Sapphire Health Services, LLC is seeking a Registered Nurse (RN) to deliver comprehensive nursing care in a skilled nursing setting in Gresham, Oregon. You will assess residents, administer medications, monitor conditions, and collaborate with an interdisciplinary team.

This on-site, full-time role requires an active RN license in the state, an Associates degree, and strong communication and documentation skills; familiarity with EHR systems is a plus.

Qualifications

• Registered Nurse licensure in the state of practice.
• Experience in skilled nursing facility, long-term care, or acute care preferred.
• Strong clinical skills and geriatric knowledge.

Responsibilities

• Conduct comprehensive assessments of residents upon admission and as needed.
• Participate in care plan meetings with residents, families and the care team.
• Provide direct nursing care including medications, wound care, and treatments.
• Monitor health status, vital signs, and responses to treatment.
• Maintain MARs and document in EHR or paper charts.
