Job summary

Sapphire Health Services in Gresham, Oregon is seeking a Registered Nurse (RN) to provide comprehensive nursing care for residents in a skilled nursing setting. You will assess residents on admission, administer medications, participate in care plan meetings, and work with an interdisciplinary team to ensure safety and quality of care.

Qualifications

• Registered Nurse licensure in the state of practice.
• Experience in skilled nursing, long-term care, or acute care preferred.
• Strong clinical skills in geriatric care.
• Excellent communication and organizational abilities.
• Ability to work with an interdisciplinary team.

Responsibilities

• Conduct admissions assessments and periodic evaluations.
• Participate in care plan meetings with residents and families.
• Provide direct nursing care, medications, wound care, and interventions.
• Monitor health status, vital signs, and treatment responses.
• Assist with Activities of Daily Living and ensure care aligns with care plans.
• Document in EHR and maintain MARs.
• Coordinate with physicians and staff for continuity of care.
