Job summary

Responsibilities

• Patient Assessment: Conduct comprehensive nursing evaluations, monitor vital signs, and track changes in patient conditions.
• Medication & Treatment: Administer prescribed medications, therapies, and clinical treatments while monitoring patient responses.
• Work within interdisciplinary Patient Aligned Care Teams (PACTs) with doctors, specialists, and social workers to design individualized care plans.
• Maintain accurate, timely health records using VA EHR systems.
• Educate veterans and families on chronic disease management, self-care, and treatment paths.
