Job summary

UnitedHealth Group is seeking a Registered Nurse for Home Health to deliver and direct nursing care in patients' homes under physician orders, ensuring compliance with laws and agency policies. You will coordinate care with a multidisciplinary team, maintaining continuity of patient care through effective communication with physicians, families, and community resources.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification preferred or able to complete within 90 days of hire.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits and ensures quality care and patient safety.
• Documents patient visits per policy and payer requirements, and completes comprehensive assessments (OASIS) accurately and timely.
