Job summary

Qualifications

Current and unrestricted RN licensure in state of practice.
1+ years of Home Health experience.
Current CPR Certification.
Ability to function in diverse home settings and meet patient needs.

Responsibilities

Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
Supervises home health aides and licensed staff, provides instruction, and assigns tasks per regulations.
Provides supervisory visits as required.
Documents and delivers high-quality clinical services within plan of care and infection control standards.
Completes comprehensive assessments (OASIS) and reconciles medications.
Experts in communication with physicians, patients, and family to ensure quality care.
Participates in on-call and weekend rotations and QA activities.
