Responsibilities

• Assist with discharge planning and follow-up throughout length of stay.
• Develop individualized care plans with the resident and care team.
• Implement plans cost-effectively, engaging resources for resident and family counseling.
• Collaborate with physicians and health professionals and share care updates.
• Maintain resident comfort and adherence to current nursing practices.
