Job summary

Interim HealthCare Inc. is seeking a Registered Nurse (RN) to provide in-home direct patient care in West Virginia. You will follow the provider's care plan and monitor progress closely, ensuring comfort and dignity for clients.

You will document vitals, administer medications, and collaborate with a care team to achieve exceptional patient outcomes. A licensed RN in the state is required, with reliable transportation and flexibility for FT/PT/PRN positions.

Qualifications

• Education: Associate degree in Nursing (AND) or Associate of Applied Science in Nursing (AAS) required.
• Licensure: Current unrestricted license to practice as an RN in the state.
• Reliable transportation to/from care sites and/or work locations.
• Practical trach and/or ventilator experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the plan of care.
• Assess and document patient conditions and report changes.
• Administer medication, insulin, and IV/fluids; document thoroughly.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Assess for further injuries, infections, bedsores and deliver treatments.
• Collaborate with providers, aides, and allied health professionals to ensure patient outcomes.
