Ascension is seeking a Registered Nurse at the Carmel Wellness Clinic in Carmel, IN to join our patient-centered team. You will implement and monitor care plans, coordinate multidisciplinary care, and document patient status while following physician and nursing orders.

In this role you will assess discharge needs, collaborate with the healthcare team, and support a smooth transition from hospital to home. A Diploma in Nursing and RN licensure are required.

Qualifications

• Diploma or licensure required to practice nursing.

Responsabilités

• Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
• Serves as a primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

Formation

Diploma from an accredited school/college of nursing

Required professional licensure at time of hire
