Job summary

Longevity Health Plan is seeking an RN Field Coordinator to blend hands-on patient care with administrative coordination in long-term care settings. The role involves traveling to nursing homes to support Advanced Practice Providers, prescreen enrollees in EMRs, and educate facility staff on our model of care.

The position requires an active RN license, a valid Florida driver’s license, and willingness to travel as part of the Market Director of Clinical Services’ assignments.

Qualifications

Licensed registered nurse in good standing.
Valid state driver’s license with good driving record and auto insurance.
Auto insurance coverage as required by state of residence.

Responsibilities

Conduct in-person visits at multiple nursing homes to assist APNs with clinical and administrative tasks.
Prescreen new enrollees using facility EMRs and identify continuity of care needs.
Oversee member appointments and ensure timely communication with providers and facility staff.
Assist with transitions of care and prior authorization requests; help members access services outside the facility.
Lead orientation sessions for nursing homes to educate staff on Longevity Health Plan model of care.
