The RN HSS Clinical Care Coordinator plays a vital role within our Integrated Care Model by advocating for members, assessing their needs, and ensuring they receive appropriate services. This is a field-based role within Worcester County, MA, offering flexibility to work remotely, with approximately 80% of time spent visiting members in their homes or care facilities.

Primary Responsibilities
  • Conduct face-to-face and telephonic assessments of members' medical, behavioral, functional, cultural, and socioeconomic needs.
  • Develop and implement individualized care strategies aimed at the least restrictive, most appropriate level of care.
  • Identify and refer members to social services, community resources, and supportive programs.
  • Manage and coordinate care plans throughout the continuum of care.
  • Collaborate with internal teams, providers, and community partners to ensure seamless care delivery.
  • Communicate essential health information to all stakeholders to ensure quality and timely services.
Qualifications

Required:

  • Current RN licensure, unrestricted
  • At least 2 years of clinical experience
  • Proficiency with MS Office (Word, Excel, Outlook)
  • Reliable internet and a dedicated, secure workspace at home
  • Access to transportation and ability to travel within the region
  • Valid US driver’s license

Preferred:

  • Bachelor’s or Master’s Degree in Nursing
  • Certified Care Manager (CCM)
  • Experience in community case management and managed care
  • Bilingual in Spanish
Physical Requirements
  • Ability to travel frequently and navigate various terrains
  • Transport equipment and remain stationary for computer work

Compensation & Benefits
