The RN HSS Clinical Care Coordinator will be a key part of an Integrated Care Model by relaying pertinent member information, advocating for the best care, and ensuring access to appropriate services.

If you reside within Worcester County, MA and surrounding area, you will have the flexibility to work remotely*. This is a field-based role, with about 80% of your time spent visiting members in their homes or care facilities. You’ll need to be adaptable, patient, and flexible in various situations.

Primary Responsibilities
  • Engage members face-to-face and/or telephonically to assess medical, behavioral, functional, cultural, and socioeconomic needs.
  • Develop and implement individualized care strategies aimed at the least restrictive, most appropriate level of care.
  • Identify and refer social service programs, including financial, psychosocial, community, and state services.
  • Manage care plans across the continuum as a single point of contact.
  • Collaborate with internal care teams, providers, and community resources to execute care plans.
  • Communicate health-related information effectively to ensure coordinated, quality care.

Required Qualifications

  • Current and unrestricted RN license
  • Minimum of 2 years clinical experience
  • Proficiency in MS Office applications
  • Dedicated, secure workspace at home with PHI protection
  • High-speed internet access and reliable transportation within the region
  • Valid US driver’s license

Preferred Qualifications

  • Bachelor’s or Master’s Degree in Nursing
  • Certified Care Manager (CCM)
  • Experience in community case management and managed care
  • Bilingual in Spanish

Physical Requirements

  • Ability to travel and navigate various locations
  • Transport equipment as needed
  • Remain stationary for prolonged periods during computer work

The salary range is $28.61 to $56.06 per hour, depending on experience and location. Benefits and incentives are also provided.
