The RN HSS Clinical Care Coordinator will be a key part of an Integrated Care Model by relaying pertinent member information, advocating for the best care, and ensuring access to appropriate services.
If you reside within Worcester County, MA and surrounding area, you will have the flexibility to work remotely*. This is a field-based role, with about 80% of your time spent visiting members in their homes or care facilities. You’ll need to be adaptable, patient, and flexible in various situations.

The salary range is $28.61 to $56.06 per hour, depending on experience and location. Benefits and incentives are also provided.
